CCM is designed to provide ongoing support for individuals with multiple chronic conditions. Our care team will work with you to create a personalized care plan, offering consistent check-ins, medication management, and lifestyle recommendations. Dr. Jared Barber earned his Bachelor’s in Biology and Master’s in Biomedical Sciences from Florida State University before receiving his Doctor of Medicine degree from the FSU College of Medicine. Having grown up in Calhoun and Liberty Counties, Dr. Barber is grateful to return home to serve the community and provide excellent healthcare.

Laura Ford earned her BSN from the University of South Florida and has extensive experience as an RN in Neuro ICU, Cardiac ICU, Oncology, Education, ER, and Pediatric ER settings. She later returned to school to obtain her MSN. Laura is proud to work and serve in the community she calls home.
