Medical Records
Every time a patient receives health care, a record is maintained of the observations, medical or surgical interventions and treatment outcomes. This record includes information that the patient provides concerning his or her symptoms and medical history, the results of examinations, reports of x-rays and laboratory tests, diagnoses and treatment plans. Medical records and health information technicians organize and evaluate these records for completeness and accuracy.
Technicians assemble patient’s health information, making sure the patient’s initial medical charts are complete, that all forms are completed and properly identified and authenticated and that all necessary information is in the computer. They regularly communicate with physicians and other health care professionals to clarify diagnoses or to obtain additional information.
All medical information regarding your hospital visits are protected for your privacy. Not information will be released without your consent, except under certain circumstances which must be with HIPAA guidelines.
Medical records hours are from 8 am to 4:30 pm, Monday through Friday. The charge for copies of Medical records is: $1.00 per page for the first 25 pages, then 25 cents for each additional page. Charge for worker’s compensation is 50 cents per page. For physician’s offices for continuity of care is no charge. Charge for CD is: $5.00. You must show proof of ID & sign a release to obtain your records.
